Registration

Show up in person at the Center closest to you on Monday through Friday from 9am to 5pm with the information listed below. If you cannot register during those hours, please call the center to make arrangements.

When you come to register your child, please bring:

• Current Medical Examination and Immunization Records
  Ask your health care provider to fill out and sign the attached medical exam and immunization form. (Note: an up-to-date immunization record is required by state.) If your child has not had an examination within past 12 months, make an appointment for him/her with your health care provider. If you do not have a health care provider, please call Child and Teen Check-up at 612.348.5131.
  
• Dental Examination (required for children 3 years and older)
  Ask your dentist to fill out and sign the attached dental exam form. If your child has not had a recent dental exam, make an appointment for him/her with your dentist. If you do not have a dentist, follow the directions on the dental exam form to make an appointment with Children’s Dental Clinics at a clinic in one of our centers, or at their downtown location. Call 612.746.1530 to schedule the dentist appointment.
  
• Proof of Income
  If you receive MFIP (Minnesota Family Investment Plan), SSI (Supplemental Security Income), SSDI (Social Security Disability Income), or Unemployment Compensation, bring official documentation of income. If you are employed, bring a W-2 form, tax return, or other income verification.
  
• Emergency Information
  Please bring names, addresses and phone numbers of two emergency contacts.
  
• Medical Insurance Card
  Please bring your child’s medical card showing your child’s medical insurance number and PMI number.
  

Full Day Head Start and Early Head Start Applications Only

• Name and phone number of your MFIP Worker or County Child Care Worker
